Saudi Arabia has issued strict new medical rules for Hajj 2026, barring high-risk patients and requiring strict health screening before pilgrims can travel.
Strict Health Criteria Introduced for Next Year’s Hajj
Saudi Arabia has announced a new set of medical rules for Hajj 2026, introducing strict limits on who will be allowed to perform the annual pilgrimage. The decision, authorities say, is aimed at protecting the health and safety of millions of worshippers who gather in the Kingdom each year for one of Islam’s largest religious events.
According to the Saudi Ministry of Religious Affairs, any pilgrim arriving in the Kingdom without meeting the required health standards will be sent back to their home country. In such cases, the cost of repatriation will be borne by the pilgrim, not the Saudi authorities or the sponsoring Hajj operator.
The ministry has also warned that doctors who issue fitness certificates to ineligible applicants will face strict action. The warning is intended to curb false or careless medical clearances that could put vulnerable individuals at serious risk during the physically demanding pilgrimage.
Patients with Serious Kidney, Heart and Liver Diseases Barred
Guidelines released by the Saudi Ministry of Health outline several categories of pilgrims who will not be allowed to participate in Hajj 2026. Among those declared ineligible are people suffering from chronic kidney diseases, including patients who require dialysis. Authorities say the intense heat, long walking distances and crowded conditions during Hajj can make it extremely difficult to manage such conditions safely.
Heart patients who cannot withstand physical exertion have also been barred from travelling. Hajj rituals often require long hours of walking, standing and climbing in high temperatures, conditions that can strain even a moderately weak heart. By excluding patients with serious cardiac issues, Saudi officials hope to reduce the risk of medical emergencies and deaths during the pilgrimage.
Similarly, people with chronic liver or lung diseases have been ruled out. Such conditions can weaken the body’s ability to cope with stress, infections and exhaustion, all of which are more likely in crowded and demanding environments like Hajj.
Neurological, Psychiatric and Age-Related Conditions Included
The new rules also cover a range of neurological and psychiatric conditions. Individuals with severe neurological disorders, psychiatric illnesses, dementia, weak memory or major physical disabilities will not be permitted to undertake the pilgrimage. Saudi authorities stress that these conditions can affect a person’s ability to follow instructions, navigate crowded spaces and respond in emergencies.
Elderly pilgrims with advanced age-related diseases such as Parkinson’s and Alzheimer’s have been specifically mentioned as ineligible. Officials argue that while the desire to perform Hajj is deeply respected, the practical risks of taking frail elderly individuals into intense heat and large crowds are too great to ignore.
In many cases, such patients require continuous assistance, medication schedules and stable environments. The unpredictable pace and demanding nature of Hajj can make it extremely challenging for caregivers to ensure their safety, particularly when moving between different ritual sites.
Pregnant Women and Infectious Disease Cases Excluded
Pregnant women have also been excluded from Hajj 2026 under the new medical guidelines. Doctors say that vigorous physical activity, long-distance walking and extreme heat can pose serious risks for both the mother and unborn child, especially in later stages of pregnancy.
The list of ineligible categories further includes people diagnosed with certain infectious or serious illnesses such as whooping cough, tuberculosis, viral haemorrhagic fevers and cancer. Saudi authorities have emphasised that these conditions not only endanger the patient but may also pose a risk to other pilgrims in densely populated areas.
By enforcing these restrictions, the Ministry of Health aims to prevent outbreaks of contagious diseases during Hajj and reduce the pressure on hospital facilities that already operate at maximum capacity during the pilgrimage season.
Medical Screening Before Departure and on Arrival
The Ministry of Religious Affairs has confirmed that medical teams will screen pilgrims before their departure from their home countries. Accredited Hajj operators and national authorities will be responsible for ensuring that only candidates who meet the mandatory health criteria receive clearance to travel.
On arrival in Saudi Arabia, inspectors will recheck fitness certificates and may conduct additional health assessments where necessary. Only those who pass these checks will be allowed to proceed to the holy sites in Makkah and Madinah.
The new rules place added responsibility on pilgrims, families and Hajj organisers to take medical requirements seriously. Prospective pilgrims are being advised to consult doctors well in advance, undergo necessary tests and be honest about their medical history. Failure to meet the criteria could result in last-minute travel disruptions and significant financial loss if a pilgrim is turned back at the airport.
While some may view the restrictions as harsh, Saudi authorities maintain that the measures are necessary to safeguard lives and ensure that those who travel for Hajj are physically capable of completing the rituals safely and with dignity.
